What to expect?
Final Benefit Payment
Claim closure letter
Claim closure typically takes place when you no longer require services, have exhausted your benefits, or are no longer eligible.
Reason 1: Recovery or Withdrawal
No longer requiring covered services due to recovery.
You voluntarily close claim because you are no longer receiving covered services.
Reason 2: Benefits Exhausted
Benefits have been exhausted under the policy.
Remaining balance is displayed in your explanation of benefits that is sent with payments.
Reason 3: Ineligible or Passed
After review of eligibility documentation, you may be found to be no longer eligible for benefits.
Policy terminates at death.
Reason 4: No Invoices Submitted
A claim may be closed if no further invoices are submitted for review.

Continue monitoring through final benefit payment.
Send claim closure letter.

Timely communication in the event of recovery or death.
